MySZV is an online 24/7 self-service portal where SZV insured can view their insurance status and access SZV online services.

Who can request a MySZV account:
•   You must be older than 18 years to request a MySZV account
•   Dependents older than 18 years can also request a MySZV account

When you submit this form, your request will be reviewed and confirmed via e-mail within 3 working days.

Information/documents you will need:
•   Personal e-mail address (NOT your work or job e-mail). Your e-mail address will be your username.
•   Picture or scan copy of your valid Sint Maarten ID or your valid Passport (any nationality). NO driver's license. (JPEG or PDF format)
•   SZV IDnr and SZV card number. You can find this information on;

o   your valid or expired SZV medical insurance card or;
o   your SZV introduction letter/e-mail or;
o   or contact SZV Customer Service.
